Help Shape the Future of St. Rose of Lima

St. Rose of Lima Catholic Community is on a journey to know, love, and serve Christ more deeply. Together, we are called to embrace Christ’s vision for missionary discipleship so our parish can thrive and continue to be a welcoming place where others grow in faith.

The first step is identifying our greatest strengths and opportunities for growth. That is where the Disciple Maker Index (DMI) comes in! This important survey invites you to reflect on your discipleship journey here at St. Rose of Lima and share how our parish can support you along the way.

The DMI is designed to:

• Invite parishioners to reflect on their journey of discipleship.

• Help parish leadership make informed decisions.

• Support parishioners in growing in their faith.

The survey will be available online from September 27 through October 25, 2026, and takes approximately 10–15 minutes to complete. You will be invited to reflect on your spiritual growth and share your perspective on our parish’s efforts to help you deepen your relationship with Christ.

All responses are confidential, and the parish will receive only a summary of results for the community as a whole. Paper surveys will also be available for those unable to complete the survey online.
